Valley Baptist Health Plans Members can take a more active role in their health by accessing health and wellness information from a variety of sources, including:

• Valley Healthy Partners a diabetes management program aimed at helping participants live a long and healthy life with diabetes. To enroll in the program

• Health Forums is a wealth of reliable health and well-being information. Health Forums is our user-friendly, free online health database. Simply go to HealthForums.com.
  
  
• Health on target is a quarterly member newsletter distributed to all Valley Baptist Health Plans members. The newsletter provides health and wellness information as well as any health plan changes or member updates. Members also have access to an electronic version of the newsletter.
  
  
• Valley Healthy Baby is a maternity program for expectant members. This program offers educational materials, an assigned nurse that answer questions and follow-up services after delivery. If you develop special needs during your pregnancy, a nurse will provide additional support. To enroll in the program call
  1-877-424-4400. Remember, it is important to enroll early in your pregnancy!
  
  
• Childhood and Adult Preventive Health Guidelines are now available. Both the Recommended Child and Adolescent Immunization Schedule and the Recommended Adult Immunization Schedule are available for download. Please call Valley Baptist Health Plans at (877) 422-4400 if you should have any questions about either schedule.
